What forms of ID are acceptable to create a USI?
When you create your USI or when someone creates a USI on your behalf, the USI system will confirm the details you have entered match exactly with those on your chosen form of ID from the list below.
- Australian Passport
- Non-Australian Passport (with Australian Visa)
- Australian Birth Certificate
- Australian Driver's Licence
- Medicare Card
- Certificate of Registration by Descent
- Centrelink Concession Cards
- Citizenship Certificate
- ImmiCard
